You are only entitled to the EHIC if you are a resident in the EU and pay taxes. If you don’t live in the EU, you can’t have an insurance card.

You can only get one if you have dual citizenship with the US and a country in the EEA or Switzerland, or in some countries as a resident. If you live in the US and that’s your only citizenship, you can’t get one.
It’s basically an exchange agreement between the social systems of European countries to provide necessary medical care. It’s not even valid for all medical procedures, mainly just illnesses and emergencies that arise while travelling. Since the US doesn’t have socialised medical care or a reciprocal agreement for European travellers, there’s no similar deal for Americans. Travel insurance from the US is recommended when coming here, and I take out insurance when I travel to the US even though I’m a citizen.
